Grace Homes - Oakridge  provides 6 private rooms (one with a private bathroom) in a residential setting for seniors and persons with disabilities. While we specialize in dementia care we also provide chronic illness and end-of-life care. We can accomodate clients who require a 2-person transfer assist including hoyer lifts and easy stands. Our rambler home is set on an acre lot nestled in a quiet residential neighborhood surrounded by mature pine trees. We offer 24/7 care provided by health care professionals and support staff specially trained to manage the unique needs of residents living with memory loss and physical decline. We focus on providing a safe, familiar and stimulating family home environment. Meals are prepared on-site by a professional chef who has 16 years of restaurant experience. Oakridge is conveniently located at the intersection of Hwy 7 and Oakridge Road in Hopkins, MN.
