Gianna Homes Sursum Corda has been providing care and services to seniors in the western suburbs since 1999.  

Our home is licensed to serve 10 residents as an Assisted Living with Dementia Care. Our mission is to provide this care with a loving kindness that respects the dignity of each resident.  With over 25 years years of experience, you will be receive the knowledge, training, love and respect from an organization that has been around with a proven track record.  Come experience the Gianna difference.

Our rambler home is located in a quiet residential neighborhood in Minnetonka. The home is created from the ground up to foster the same feelings and experience you get when living in a real home - your own home. All our resident rooms are on the main level, and the lower level opens to a beautiful, patio and garden area with walkways, a fountain and abundant wildlife.  Our home has a high staff to resident ratio with a full-time on-site Registered Nurse to meet the needs of the residents and provide oversight and supervision.

We serve residents with Alzheimer's Disease, Lewy Body Dementia, FTD and other dementias.  We can accomodate a 2 person transfer, hoyer lift and hospice care.
